LOTO shows up on OSHA 10 General Industry and every OSHA 30 GI mock because stored energy kills people who thought the disconnect was “off.” Construction crews hit the same idea around electrical lockout and equipment servicing. The sequence that practice items hammer: prepare, shut down, isolate, apply lock and tag, verify isolation by trying to start, then work. Skip the try-start step and you fail the question — and you can fail the person in the machine.
Authorized employees perform lockout. Affected employees operate the equipment in production and must be notified. Group lockout uses a hasp or lockbox so every person has a lock on before anyone works. Shift change needs a procedure so a lock is never left with nobody accountable.
